CONGRATULATIONS to the team on becoming #LinkWorkerDay23 Social Prescribing Partnership of the Year WINNER!
The National Association of Link Workers (NALW) panel of judges were blown away by their work and have shared glowing comments, “an excellent example for integrated care systems on integrated team working, bringing together partners to deliver proactive social prescribing to improve health and well-being. An innovative model for secondary care to learn from; Social Prescribing Link Workers are based within a hospital Emergency department offering social prescribing service to all patients, including paediatric patients, to transform healthcare for all and address major challenges currently facing our health system."
